You will see the chamber fill with smoke from the bowl. Keep slowly pulling until the bottom of the glass bottle is just below the surface of the water, then take out the bowl. 

The 19mm joint can then be used as a mouthpiece. As you inhale through the mouthpiece, lower the bottle back into the chamber to push a pressurized flow of smoke up through the downstem. The inner glass will not clink or scratch against the outer glass thanks to a rubber seal lining the base of the inside bottle.
